RADIANCE PROOF PROJECT PARTICIPATION AGREEMENT

The Radiance Proof Project ("Project") is a guided four-week skincare experience where selected participants use the Doctor Hopiola Radiance Set and document weekly observations through structured photos, videos, and written reflections. Participants selected for the Project will receive a four-week supply of the Radiance Set at no cost.

Participant affirms that:

• They are at least 18 years of age.
• They do not have a known allergy to bees or bee-derived ingredients, including but not limited to propolis, honey, or beeswax.
• They do not have an allergy to any of the ingredients in the products as detailed in the ingredient lists.
• They are able to safely participate in a skincare program using topical cosmetic products.
• They can commit to four consecutive weeks of documentation and daily product use.

Company reserves the right to decline or remove participants at its sole discretion.

Participation in the Project does not require a financial investment. Company covers the cost of the Radiance Set provided to accepted participants.

Participant is responsible for completing all weekly check-ins and following program requirements.

If accepted into the Project, Participant agrees to:

• Use the Doctor Hopiola Radiance Set exclusively for the entire four-week program, except for their regular cleanser and sunscreen.
• Pause all other skincare products, including moisturizers, serums, oils, retinol, vitamin C, acids, exfoliants, or active treatments.
• Complete all weekly check-ins, which include:
  • Four required photos (front, left, right, upward angle)
  • One short weekly video update
  • Brief written responses
• Maintain consistent lighting, angles, positioning, and natural expressions in all media submissions.
• Submit honest, timely, and complete information each week.
• Follow usage instructions provided by Doctor Hopiola.
• Notify Company immediately if irritation or concerns arise.
• Refrain from making medical claims or interpreting any recommendations as medical advice.

Participant acknowledges that the purpose of the Project is to document genuine consumer experiences and generate user-generated content ("UGC").

In exchange for participation, Participant grants Company:

• A perpetual, irrevocable, worldwide, royalty-free license to use Participant's submitted photos, videos, written reflections, quotes, and testimonial statements ("Content") for marketing, advertising, social media, website use, educational materials, or other promotional purposes.
• Permission to edit, crop, enhance, or format Content as reasonably needed without altering the truth of the representation.
• Permission to identify Participant by first name, initials, or remain anonymous. Company will default to anonymity unless Participant elects to be tagged.

Participant waives any right to review or approve finished materials.

Participant understands that:

• The Radiance Set contains natural cosmetic ingredients, including propolis.
• Cosmetic products may cause irritation or reactions in sensitive individuals.
• Experiences vary and are not guaranteed.
• The Project is not intended to diagnose, treat, cure, or prevent any medical condition.

Participant agrees to monitor their skin, discontinue use if needed, and seek medical attention as appropriate.

Participants who complete all program requirements will receive:

• A $100 discount code for a future purchase of the Radiance Set
• An invitation to apply for the Doctor Hopiola Ambassador Program

Completion is defined as submitting all required photos, videos, and check-ins for each of the four weeks.

Company may withhold the reward if Participant does not fulfill every requirement.

Company may remove Participant from the Project at any time for reasons including, but not limited to:

• Missed check-ins
• Non-compliance with skincare restrictions
• Incomplete submissions
• Inappropriate behavior
• Safety concerns

Participant may withdraw at any time, understanding that doing so forfeits the completion reward.

Participant information will be securely stored and used solely for Project administration, communication, and improvement.

Company will not sell or disclose Participant's personal information outside of these purposes.

To the fullest extent permitted by law, Participant agrees that Slate & Lilly LLC, its owners, employees, affiliates, and contractors:

• Are not liable for allergic reactions, sensitivities, or adverse responses,
• Are not liable for misuse of products or deviation from instructions,
• Do not guarantee specific outcomes or timelines, and
• Are not liable for damages arising from participation in the Project.

Participant releases Company from all related claims.

This Agreement is governed by and interpreted under the laws of the State of Arizona, without regard to conflict-of-law principles.

This Agreement, including the Addendum below, constitutes the entire understanding between Company and Participant and supersedes any prior discussions or representations.
